Your 45-day free home trial will be honored for 45 days starting from the date we ship your hearing aid. The hearing aid becomes yours at the moment we ship it. Both parties agree that the sale occurs in Illinois and that Illinois law shall govern the making and enforcing of this purchase agreement. Lloyd Hearing Aid Corporation fully complies with all governmental regulations as stipulated by Illinois state law, FDA, FTC, and the laws of interstate commerce.
Waiver to Medical Evaluation Requirements:
I have been advised by Lloyds that the Food and Drug
Administration and the State of Illinois have determined that my best
interest would be served if I had a medical evaluation by a licensed
physician, preferably a physician who specializes in diseases of the
ear, before purchasing a hearing instrument; or a test by a licensed
hearing aid dispenser utilizing established procedures and
instrumentation in the fitting of hearing instruments. I am 18 years old
or older and voluntarily accept this waiver which indicates that I do
not wish a medical evaluation or test before purchasing a hearing
instrument. I agree to pay for merchandise ordered or to return it in
original condition at the end of the 45 day trial period.
Note: Ordering a hearing instrument from this web site constitutes
your acknowledgement and acceptance of this "Waiver to Medical
